First of all, I am surprized that the Cryonics Society of Michigan 
has not instituted some of the measures you list. Anyone who considers
the issue of cryonic suspension would add that none of these measures
really guarantee that you will be suspended, but that is a weak 
reason not to do them anyway. 

One simple thing you might do is to ask Alcor for its membership
forms. Yes, I know there has been some hostility in the past, but
that should be disappearing, as it should. It's NOT that you
would simply copy those forms, but that you could use them to
write out forms for the Cryonics Institute. Those forms will
inevitably be different, because some of the policies of the two
societies differ. But they would help you write out forms for
the Cryonics Institute.

Years ago, when I first lived in Australia, I did a lot of work
to set up CAA as a society incorporated under Australian law.
Getting forms and other legal information for the other societies
helps a lot: that is one major advantage of cooperation.
